Anterolateral meniscofemoral ligament with congenital aplasia of the anterior cruciate ligament: a case report

Abstract
A 34-year-old male patient who had chronic anterior instability of the left knee without a history of trauma was referred to our hospital. The anterior instability was confirmed by physical examination and anterior drawer stress radiography. Further examination by magnetic resonance imaging revealed an absence, or aplasia, of the anterior cruciate ligament (ACL), and a result that closely resembles a bucket-handle tear of the lateral meniscus. Arthroscopically, we found a cord-like structure that originated from the anterior horn of the lateral meniscus and inserted to the medial wall of the lateral femoral condyle. Again, the ACL was not observed. In this report, we describe an extremely rare case of anterolateral meniscofemoral ligament (ALMFL), which had been previously reported only once in the literature, combined with aplasia of the ACL. We performed ACL reconstruction using a single 4-strand semitendinosus tendon autograft. At one-year follow-up, the patient could perform daily activities without any discomfort.
